Dependent verification process
SAIF requires employees who enroll their eligible dependents (spouse, domestic partner, children) to their health plans to provide documentation for each dependent.

Please "share" or email the required document(s) to Benefits@saif.com.
- Review the List of acceptable documents so you know what documents to provide. [PDF]
- If you need to order a required document and it takes longer than 30 days to arrive, please submit the receipt and then provide the documentation once it arrives.
- If you enroll a spouse or domestic partner on your medical plan, you will also need to complete the Employer verification form.
- If you enroll an unregistered domestic partner, you will also need to complete the Unregistered domestic partner form.
- If you enroll a spouse or domestic partner, registered or unregistered, you will also need to provide a piece of joint documentation.

The CDC website (http://www.cdc.gov/nchs/w2w.htm) can help you with ordering those documents. There may be non-reimbursable costs associated with obtaining new copies.
