Dependent verification process
SAIF requires employees who enroll their eligible dependents (spouse, domestic partner, children) to their health plans to provide documentation for each dependent.

- If you enroll a spouse or domestic partner, you will also need to complete the Employer verification form
- If you enroll an unregistered domestic partner, you will also need to complete the Unregistered domestic partner form
If you don’t have a required certificate/document, please order it immediately.
The CDC website (http://www.cdc.gov/nchs/w2w.htm) can help you with ordering those documents. There may be non-reimbursable costs associated with obtaining new copies. If you must order a required document and it takes longer than 30 days to arrive, submit the receipt to benefits@saif.com and upload the documentation into Workday once it arrives.
