Who is eligible?

You must be a SAIF employee, or the employee’s spouse, registered or unregistered domestic partner, dependent child under the age of 26 (or over age 26 if child is incapable of self-sustaining employment due to a physical or mental disability), dependent child under the age of 18 for whom the employee, spouse, or domestic partner is a court-appointed guardian, or dependent child for whom the employee has a qualified medical child support order.  

Employees and dependents enrolled in the Providence medical plan can continue care beyond the 12 free visits with their Lyra network provider and access medication management support if needed. These sessions are billed through Providence and are subject to in-network outpatient mental health copays. If you have Kaiser, Lyra will do a warm handoff to locate a new counselor. Kaiser cannot support care beyond the 12 free Lyra visits at this time. 

To align with HIPAA compliance, Lyra may request proof of guardianship in certain scenarios. An example of this would be if a step-parent were to reach out to establish care for their step-child, since step-parents are not automatically considered to be legal guardians of their step-children.
